Hello Doctor,
I am writing to you to request a medical
letter
from your side that states my recent illness. It would be very helpful if you could include the seriousness of my illness in detail. This letter
is to be submitted to my employer at my workplace requesting for an additional week of rest at home.Might I request you to include the symptoms that I have during my sickness

The severe headache that I am facing has only started becoming worse. Nevertherless
, my swollen red eyes
has
also been giving me pain whenever I blink or close my eyes to
sleep
which makes my sleep disturbed too, I am currently placing a cold pack on my eyes which has brought some relief to it over the past week.
Could I know the process of obtaining a medical
letter
from you stating all the above mentioned
distrubances
that I have been undergoing?.
